NOTICE TO PARENTS AND CONSENT FORM The VBS program is a faith-formation curriculum that we will be offering in our church this year. It is published by Augsburg Fortress (the publishing house of the Evangelical Lutheran Church in America). The VBS program includes a Web site located at www.thevbsplace.org (the “Web site”) where VBS learners and leaders register their personal information. These learners and leaders may also access games and other material to enrich their VBS learning experience. In order for the administrators and leaders of the VBS program to register VBS learners, communicate with those VBS learners, track attendance in the VBS program, and schedule VBS program activities, we would like you to authorize your learner to submit certain personal information to the Web site. Alternatively, you may authorize a volunteer to submit your learner’s personal information on his or her behalf, or submit your learner’s personal information yourself. For your learner’s protection, Augsburg Fortress has asked us to obtain the your consent before we give your learner access to the Web site and ask them to submit their personal information. Please take a few minutes to visit the Web site at www.thevbsplace.org. Be sure to review the Terms of Use, the General Privacy Policy and the Children’s Privacy Policy. These documents are posted at the bottom of eachof the Site. If you do not have access to the Internet or, for any other reason, would like us to provide a page paper copy of these documents, please call Augsburg Fortress Customer Service at 1-800-328-4648. We will be happy to mail a paper copy to you. CONSENT: I have read and understand the Terms of Use, including the General Privacy Policy and Children’s Privacy Policy. I hereby give my consent for my learner to use the Web site and to submit his or her personal information to the Web site. I agree that in return for access to the Web site, my learner will comply with and be bound by the Terms of Use, the General Privacy Policy, and the Children’s Privacy Policy. I hereby release Augsburg Fortress and its affiliates from any and all liability arising out of my learner’s use of the Site.
